Certificate of Insurance Request

A certificate of insurance is a document provided to an outside organization indicating evidence of 91¶ÌÊÓƵ’s insurance coverage.   Outside organizations typically request a certificate of insurance when the University is holding an event or activity at an off-campus location and there is a written contract which requires insurance.
To request a certificate of insurance contact us at least 7 business days prior to the certificates due date along with a copy of the contract so that the specific insurance requirements can be met.  If a contract is not available, provide contact information and answer the following the basic questions that describe who, what, when, where, why, and how.
